Person-Centered Care Model

Personalized Care to Nurture the Spirit
Behind every resident lies a spirited passion for something. At Eastridge, our caregivers nurture that passion by fostering a trusted relationship through person-centered care.
Our specialized and state-approved dementia care training program goes beyond the basic requirements and equips caregivers with the expert knowledge needed to get to the heart of the matter.

Each of our dementia trainers has been carefully selected and is a graduate of our company’s Train the Trainer program facilitated by a certified dementia consultant. Continually learning new techniques in caregiving for those living with dementia, our trainers provide caregivers with the tools needed to establish heart to heart connections through person-centered care.

OUR PHILOSOPHY IS PERSON-CENTERED CARE

When seniors transition from their home to a community home, the loss of independence and being out of their comfort zone can be tough for some. To make the transition easier, our philosophy at Eastridge encourages choice, purpose, and meaning in residents’ lives – helping them to feel valued. Our passionate caregivers dedicate themselves to building a trusted rapport with residents though encouragement, praise, conversation, and by getting to know each person – their personality, children, careers, likes, and dislikes.

We focus on building upon the strengths and abilities residents have and encourage them to be as independent as possible – from taking a leisurely stroll outside to dancing the waltz with a partner. No matter the level of cognition or physical ability, we provide engaging activities that stimulate both the body and mind. This independent lifestyle combined with our progressive care program, helps residents be the best version of themselves. With Eastridge’s person-centered care, residents experience an improved quality of life in a spirited community they can thrive in.
